Jose’s Story

๐ŸŽ“ Clinical Background

Jose graduated from WesternU with his PharmD in 2023 and completed a community-based residency with training in both ambulatory care and community pharmacy. Much of his work has been with underserved patient populations โ€” including those experiencing homelessness โ€” helping bridge gaps in access to care. He has also worked in IV home infusion, but ambulatory care and chronic disease management is where he has found his real passion: working closely with patients to help them manage their conditions and navigate the healthcare system.

What drew Jose to chronic disease management was seeing how many patients fall through the cracks โ€” especially in underserved communities, where clinic visits are short and focused on the immediate problem. Patients with diabetes or hypertension don't always get the ongoing support they need to manage their health day to day. He wanted to work somewhere with time to build real relationships, answer real questions, and help patients feel like someone is truly in their corner. Gojji makes that possible.
